The Sixth Annual as Golf Tournament will be held at the Canyons at Blackjack Ridge Golf Course, Sand Springs, OK,
On Monday, April 19th.
Fee: $110.00 per person or $440.00 per foursome

All registrations must be received by April 7th, 2010 Individual golfers will be assigned a team by the golf committee, NO CHANGES WILL BE MADE AT THE GOLF COURSE THE DAY OF PLAY. THERE WILL BE NO REGISTRATION AT THE GOLF COURSE, YOU MUST REGISTER AS AN INDIVIDUAL OR TEAM BY APRIL 7th, 2010. PARING OF TEAMS AND INDIVIDUALS WILL BE FINAL. NO TEAM OR INDIVIDUAL WHO HAS NOT PRE-REGISTERED WILL BE ADDED AFTER APRIL 7th, 2010. If a golfer who is registered can not attend, a substitute golfer may take the place of the original golfer, but the team assignment WILL NOT CHANGE. Entry fees will not be refunded for any reason other than closing of the course by the PGA
